As I understand it, there can be issues with the auto boxes on these. What exactly is the issue with them? What's the main reason for failure and most likely cause?

Probably the biggest killer of most slushboxes is not changing the fluid.
Followed closely by nailing it in 1st. Honda uses motorcycle-style multiplate clutches on a regular twin-shaft cogbox, instead of an epicycyclic, whose brake bands usually tend to be slightly tougher. But will die just the same, if you think your name is Bullitt.

Yer its just triptronic selection adds complications to an already comlicated autobox. They tend to be weaker selectors and break.
The autoboxes wastes 25% power where the manuals use around 15% as a general rule.
They are good when they work but just get a manual IMO plus you can get the U2Q7 LSD box if you want and stick that on from an ATR. Unless you get a Type S and you cant.

as been said service is killer, in my experience they are not weak, i servicing one prelude with autobox, over 160 th on clock , service done only by honda and last 1 year by me with proper fluids and correct service, box feel fantastic.
i have few customers with preludes 3-4-5 gen. manual and auto and if service is as it should be done by people who understand this cars they go for years without any problems

I guess just like everything else, with improper use and poor servicing, a component is going to fail.
There is no knowing how well a former owner has used the box so this is probably the main fear for buying one.
My SiR, which is for sale
, has an auto box and with 100,000 miles now feels just as new. Of course I change the fluid regularly as per the service interval using the correct fluid, never use the manual select when cold, change into neutral at lights etc and as said above by Jozefsan, no reason why it shouldn't continue to work for 100,00 miles yet
