Any holes in the exhaust, specifically before the O2 sensor?

I would have a quick look at the flexi pipe

Sometimes a leak is not apparent untill you get down on the floor and someone blips the throttle.

Mine does this too from time to time; drops to about 500rpm for a couple seconds then picks back up to 800rpm and settles. It doesn't hunt, only drops down as the revs drop after it's been taken out of gear...

Is there an IAVC on the ATR TB?

The IACV is on the intake mani. The stock ATR TB has no FITV

The IACV is bolted right on the front face of the intake manifold, behind the fuel rail. Held on with two bolts and has coolant running through the bottom of it. It is likely to be the cause of your troubles. They get carboned up.
