The 5th gen manual lists the clearance between the nub of the sensor and the ring as 0.4 to 1mm only. looks like it might be another couple of millimetres on top of that in your picture.
If you can file the bolt off and remove any surface corrosion, lube it up and tap the sensor in so it sits flush - I recon that will fix the issue as if the sensor is too far away, it just wont see the wheel turning hence the wheel lock code you are getting.
Congratulations to vtecmec for winning May/June's Lude Of The Month, with his DIY Turbo BB1 build.

4th Gen - Strange 4WS Issue
This could be the only thing causing the issue as everything else checks out. I’ve sent the car into Matt @ Red-R Performance (Daventry) to remove the broken stud so the sensor can be properly installed. At the same time he will do a service on it including air con service.Scott560 wrote: ↑Thu Oct 24, 2024 7:16 pmThe 5th gen manual lists the clearance between the nub of the sensor and the ring as 0.4 to 1mm only. looks like it might be another couple of millimetres on top of that in your picture.

I’ll update this post once I get the car back.

Sorry, only just caught up with all this.
Yup, agreed - if there is excess gap on the ABS sensor that would almost certainly cause the signal to be too weak/non-existent and that would flag as a fault.

4th Gen - Strange 4WS Issue
Update:
Hi guys,
Just wanted to give you an update on the issue. I can confirm the issue has now been fixed!
I ended up replacing the rear hub. I’ve driven it for about 60 miles and thankfully no fault codes. I had the wheel alignment done by a specialist as it was weaving all over the road after the 4WS was working correctly. All four corners needed alignment. The previous owners had it aligned last year thinking it would resolve the 4WS issues.
The dealership also disabled the original Honda alarm system as it would no longer lock the doors with the remote. After replacing the slave lock actuator, that is also got fixed (cost less than £10 from Amazon) and oddly identical to the broken one. So I also now have working remote central locking alarm.

It’s been serviced inc air con service, looks and drives like brand new. Over the moon and a great result to end the year with.
Have a great Christmas and New Year guys!
