He may have been coked up but as Becca said, this is a more typical reaction from salts. May have got some coke filled out with salts.

As for why the face, there are multiple reasons. One is because there are lots of nice protrusions to get a bit on (nose, cheeks, ears, chin, and eyes if you pop them out), and another is that it is the key component of someone's outward identity, so eating someone's face can almost be seen as consuming their identity.

but mental illness probably paid a part in these types of attacks, some major wires loose

One of those unfortunate slips through the system by the looks. No system can ever be perfect, so this sort of stuff will happen from time-time, but as usual no-one will have the bollocks to say that and instead we'll get the usual "we must make sure this never happens again" bollocks spouted all over the place...
