engine cuts out after starting

ironically ive been out to the car on various occasions and cant replicate it. since i first posted this thread, i had a call from her saying she took 4 attempts of starting before it stabilized enough to rev the engine.

here is my old engine bay, identical engine.

red arrow is the iacv.. is the item pointed out in green the fitv?

I still can't believe how imaculate and spotless that engine bay was Sam...

i know, i spent ages on it, i then decided to fit some bonnet tilters and it got bloody filthy in 3 months, so had to do it all again.
